How Does Medication-Assisted Treatment Work??

Medication-assisted treatment works by combining the use of medication with therapeutic services. FDA-approved medications, which are prescribed by an expert care team, can alleviate the physical effects of withdrawal. At the same time, individual and group therapy sessions led by certified behavioral health professionals can provide a supportive structure for patients as they address the mental and emotional effects of addiction and work toward a substance-free future.  

Opioid use affects brain chemistry and can change how a person’s brain works overall, which is why people often experience intense cravings if they try to stop using opioids on their own. MAT medications can help people safely manage these cravings as well as other symptoms of opioid withdrawal. Although the medications prescribed as part of MAT can vary depending on the location where one receives care, there are a few that are commonly indicated for treating opioid use disorder: 

  • Methadone: Methadone is a full opioid agonist, which means that it fully activates the same brain receptors as illicit substances like heroin and prescription pain medications like oxycodone. However, methadone is administered in a controlled dose that does not produce the same “high” as these substances. Instead, methadone tricks the brain into thinking that it has the drug it craves. Because it is a long-acting medication, it can keep the brain stable throughout the day. 
  • Buprenorphine: Buprenorphine is a partial opioid agonist. This means that it activates the opioid receptors, but only partially. While this partial activation is enough to reduce cravings, buprenorphine produces less of a “high” than opioids do. The medication also has a “ceiling effect,” which means that taking more of it will not increase its effects. This “ceiling effect” lowers the risk for misuse and overdose. 
  • Vivitrol: Vivitrol is a long-acting injection of naltrexone, an opioid antagonist that blocks opioid receptors in the brain. If someone uses opioids while they are taking Vivitrol, they won’t feel the “high” they normally would. In other words, Vivitrol does not directly address active withdrawal symptoms. Instead, it works best after someone has already detoxed and wants to avoid future relapse. 
  • Suboxone®: Suboxone® is a combination of buprenorphine, a partial opioid agonist, and naloxone, an antagonist that completely blocks the brain’s opioid receptors. While buprenorphine can ease the discomfort associated with withdrawal, naloxone is added as a safety feature. Naloxone stays inactive when it is taken by mouth as prescribed. However, if someone tries to inject Suboxone® to get high, naloxone kicks in and blocks the euphoric effects of buprenorphine, which helps prevent misuse.

  • Ease withdrawal symptoms and cravings: FDA-approved medications like methadone, Vivitrol, buprenorphine, and Suboxone® can help people more easily manage withdrawal symptoms and can reduce intense cravings.  
  • Improve engagement in therapy: With fewer physical and mental distractions, patients can focus more fully on the counseling aspect of treatment. Stronger engagement in individual and group therapy sessions can promote lasting recovery. 
  • Reduce the risk for future relapse and overdose: MAT significantly lowers the chance that a person will return to opioid use and can protect against potentially fatal overdoses in the future.
